From 2012 to 2013 she starred as Daniella Devlin in the TV series The Mob Doctor. Since 2011 she has had a recurring role as Dr. Rachel Cranston on the series NCIS and starting in 2014 she starred in the series Alpha House. Makkena is also known for playing the role of Sister Mary Robert in the movies Sister Act and Sister Act 2: Back in the Habit. In 1994 she was nominated for a Q Award for her role at the Viewers for Quality Television Awards for NYPD Blue. In 2019 she appeared in the film A Beautiful Day in the Neighborhood alongside Tom Hanks.
